Home and Community Based Care Symposium 4/25/16

This is a symposium designed for health professionals and community members to learn more about the forces shaping healthcare delivery and discover how home and community based care models are also changing.  The symposium and discussions will provoke new perspectives on, and frameworks for, home and community based care across the life span.  Attendees will learn with and from leaders in healthcare policy, education, practice, delivery systems along with healthcare practitioners and community members.
